My 6 montha Cocker Spaniel had some lesions raised crusty spots in the arm pit, help?

Hi friends, my 6-month cocker spaniel had some lesions --raised crusty spots--in the arm pit and now shows some on his neck also--he scratches these spots. Is this something to e worried about? My concerns and monthly visits to his vet have begun to amuse everyone at the clinic. Also, the puppy begins to wheeze during his bath, which is why he gets bathed only once a month. The vet attributes this to the smell of the shampoo. Is there an alternative to shampoo?--I use Cisaflux. Would greatly appreciate any suggestions.
